in the Run box enter firefox.exe -safe-mode
the firefox should start with all the extensions disabled. and if you think the problem was caused by the firefox's download manager then try running the same task as before and with Windows task manager opened, and check the process again?
if that helps. this means one of the extension is causing the problem... no need to uninstall the firefox program files, but you'll need to create a fresh profiles for the firefox and then try installing your extensions one by one and check it after each extension has been installed... this to see which extensions was the actual cause... then just don't use the extension.
if that did not help...,
with firefox is already closed, go to your firefox profiles folder,
for a typical installation of the firefox your profiles folder is most likely to be at \doc and settings\your Windows log on name\app data\mozilla\firefox\profiles\xxxxxx.xxx,
and then copy the prefs.js (and user.js if it does exist) to your desktop and then delete the file(s) in that folder.
now, start the firefox in its -safe-mode again (for now, all of the previous firefox and its extensions settings will be lost) and run the same task again to see if any of the previous settings have causing this problem?
post back what you have found out on these.
