MSI 5950 ultra heat?

Hi guys...
I have a 5950 Ultra that was RMA'd twice, supposedly replaced the second time, that suffers from gradually increasing visual artifacts. These range from the odd pixel flashing to utterly unwatchable to wireframe while playing any 3-d game. happens on 2 differant systems, 1 an AMD/Nforce2 combo, the other a P4/Intel 875 setup. Both system have at least 350 watt P/S. I've noticed the rear fan seems wobbly, but wonder if it might help to remove the heat sinks, and re-apply some thermal compound. The damn thing is driving me nuts, and I'm not getting much love from MSI. Oh, and BTW, it's never been O/C'ed, underclocking it helps some.
Any ideas?

Obviously a heat problem, as you suggested. If the fan is screwed up, then you should get a replacement. DO NOT relent until you get a fully working card. Don't remove the sinks and fan until you know FOR SURE that they wont take it back (which im sure they will replace it) as this will void your warranty.
