Sometimes the RAMDACs or the capcitors on the video card can make a whinning sound when power is run through them. It doesn't mean that the caps are bad though, but I've seen (heard) this happen with certain brands of capcitors and video cards (MSI boards in particular do this). Over time they can start 'whining' for no reason. If you are worried about it thouhg, and the card is still under warranty, I would suggest talking with Sapphire about it.. and about sending it in for repair/replacement.
Another thing to consider (although this doesn't sound likely if the card only makes this noise when you have the monitor on) is that the fan sequence for most ATI X8xx series video cards is that they will start at full speed during initial bootup, and slow down to a lower speed there after. It could be that the fins on the fan are very close to the outer plastic casing, and are scraping against them. However, having said that, it could be that the fan is going (most companies use brushless fans, and they tend to seize up fairly quickly from dust). I would give the fan a good cleaning, or consider replacing it with a Artic Cooling ATI Silencer or some other cooler like one from Zalman.
