Volume control is not done in Surrounder+, it is done in epilog.
It sounds like the mapping may not be correct for your A4. Did you try adjusting the volume of the Digital Outputs to see if any of them effect any of the analog speakers?
I would do the following:
Reset AC97 (AC97 page of kX Mixer).
Set Surrounder+ to 2.0 Stereo Only.
Play some looped audio (make sure audio player is set to use kX Wave 0/1).
Set the Master Volume such that it is audible.
Set all speaker volumes (analog and digital) on Ins and Outs page to 0 dB.
Make sure Swap Front and Rear and Route HP to Ctr/Sub are disabled.
Disconnect all connections from Surrounder+ to epilog in the DSP.
Verify the mapping of epilog's inputs to the speakers.
i.e.
Connect Surrounder+ fL/fR to each pair of pins on epilog (one pair at a time) and note if/where you get sound. When you do get sound, take note of the name of the pins that you are currently connected to (on epilog), and try adjusting the corresponding slider (the slider that goes with the pin names) on the Ins and Outs page of kX Mixer to see if it is able to control the volume.

@mayonitrof
What is the specific model of the Audigy?
Check to see if it is listed on the kX Soundblaster Compatibility List.

Audigy4 Epilog Mappings
@Russ
Thanks much for the quick reply! Ahh, yes, I see that Surrounder+ is what I was calling Speaker Setup, and epilog is what I was calling Ins 'n' Outs. I believe the Audigy4 problem I'm experiencing is with epilog, rather than Surrounder+. Tests results are as follows:
I've tried adjusting all of the outputs in the Ins 'n' Outs. You're right, the Generic S/PDIF Out does control my rear (green plug) speakers! However, it controls the green plug whether the front and rear speakers are swapped or not. I'm not sure, but I had thought that swapping the front and rear channels would swap which speakers are controlled by the Front or Rear (or even S/PDIF) outputs in the Ins 'n' Outs page. Do you know if that is usually the case? (Or are the epilog controls wired to specific outputs of the sound card, ignoring which channels are going where?)
I went through the steps you recommended, and here's what I found:
The Left Rear (green) jack on the Audigy is mapped to: "Generic SPDIF Left [4002] out2" (the Ins 'n' Outs fader works properly)
The Right Rear (green) jack on the Audigy is mapped to: "Generic SPDIF Right [4003] out2" (the Ins 'n' Outs fader works properly)
The Frong Left (black) jack on the Audigy is mapped to: "Analog Rear Left [4008] out8" (the Ins 'n' Outs fader works properly)
The Frong Right (black) jack on the Audigy is mapped to: "Analog Rear Right [4009] out9" (the Ins 'n' Outs fader works properly)
The Center or Headphone Left (orange) jack on the Audigy is mapped to: "Digital Center / SPDIF1 Left [4004] out4" (there doesn't seem to be an Ins 'n' Outs fader for this epilog pin
The Subwoofer or Headphone Right (orange) jack on the Audigy is mapped to: "Digital SubWoofer / SPDIF1 Right [4005] out5" (there doesn't seem to be an Ins 'n' Outs fader for this epilog pin
You can call me crazy, but I have a sneaking suspicion that might be related to my inability to control the volume of my center channel and subwoofer channels ^_^
I think I might know a way around my problem now, I could probably just connect the center and subwoofer outputs of Surrounder+ to the epilog inputs for the Rear Channels, and then control my center and subwoofer as if they were rear left and right. I hope this information helps out in either event, please let me know if I can do anything else to help the kX project!

Yes that is the case, but swapping Front and Rear (via kxmixer) only effects the analog outputs, and because the mapping is off with your card, the front is not swapped correctly.
There are a couple of workarounds you can do, until Eugene is able to correct the mapping.
1) Insert a GainHQ in between Surrounder+ and Center/LFE (SPDIF1 Left/SPDIF1 Right), and use it to adjust the volume of those channels.
-or-
2) Edit the kX skin to reassign which kX Mixer sliders are associated with which registers in epilog.

DSP Effect Window Location on 2 Monitors
Hello,
Using the 3538m drivers, whenever I close an effects window (Peak, Oscilliscope, MX8) it will forget it's location on my 2nd Monitor.
If I close the effect windows on my primary monitor they will remember their position properly, but if they were closed while on my secondary monitor, they'll re-open front and center on the primary.
Thanks much!

it seems renaming plugins - need limiting of how many characters you can use - too many with the info plugin - and it crashed kX mixer.
Reported here:
Info plugin renamed too long makes kXMixer crash

Kx ram issue
Here's an intresting one..
Running Vista32 (2GB ram, Core2duo), and everything was fine.
I bought another 2GB ram (so 4GB total), and kx driver no longer produces sound.
Can sometimes hear sharp static sounds when there should be sound playing (sounds like the sound card is taking a random chunk of memory, and playing the contents as audio).
Speaker test gives me "Failed to create sound".
If I limit the amount of RAM accessible to Vista to 2gb or less using msconfig boot options, then sound starts working again. Anything over 2 seems to fail.
I dual boot with XP, which does not exhibit the same issues. It registers (or rather addresses) 3.5GB of ram and plays sound fine.
Have tried driver L and M, both act the same.

seems the kX speaker test applet will freeze VU displays in WinXP (full test or separate speakers)

Since I reinstalled in January I wound up using 3538m just for kicks, rather than 3537 (which I was using previously). I've been unable to use the Mic input directly in a windows application without having a complete system lock (no BSOD, just frozen screen and nothing responsive).
I'm using an older Dual Xeon system with HT disabled. Tried several PCI slots, same result. The card shares the PCI bus with an RME HDSP PCI card as well as the usual complement of onboard devices (Firewire, USB, PATA IDE, 2x SCSI etc) and under ACPI it shares irq 19 with a USB 1.1 port. I have had no playback issues with the card at all, under audio production applications, with games or general 'windows' playback (winamp, browser plugins etc). Only when I select the mic input under AC97 and attempt to access it from an application, or I've noticed even when I have the mic input selected as the AC97 source. Currently I route in the DSP routing app to output 3/4, then into a small outboard mixer which goes into my main mixer, then through some dsp cards on another machine, then via AES (pro spdif connection) back to my RME Hammerfall and that's what I select as record input in games & skype etc.
Thinking about switching back to 3537 just for kicks...

kX Soundfont loader accepts numbers up to 250 for the target bank (even though it reports (when entering a number above 250) that valid values are only from 0 to 127).
