strange.... looks like a corrupt installation.... Try and do a complete.. COMPLETE uninstall of ALL ATI related software/drivers/installations.... use a program like Driver Cleaner to completely clean up your system of any residue ATI stuff.... then try redownloading the drivers.. and installing...

Thanks for the suggestion but that didn't work. I gonna try putting it in my other system to see if it does it on there then I will know whether its software or hardware..
Edit.. Tried it in my other computer and it worked fine. I am totally confused now.. Any other suggestions? I've completely uninstalled the drivers went into safe mode ran driver cleaner cleaned up all the old ati files and still doesn't work..

Errr I just reloaded about a month or so ago..I really dont want to do it again.

i know the feeling....
You could attempt to REINSTALL your chipset drivers for your computer.. or doing a windows XP Repair...
Possibly download an older set of the drivers... or better yet
TRY Omegas drivers..... simply uninstall ONLY the ATI Control panel from the ADD REMOVE... before installing the omegas.... with the omegas.. you simply install overtop... overwriting anything
And if there appear to be still some problems.. just go to the start menu and omegas... and do a RE INSTALL of the omega drivers.. and overwrite again.. this will usually do the trick.. if it doesn't.. you might have to look at fresh install of windows...
Also.. here's a good suggestion:
to make things easier in the future... (i'm assumeing you have one hardrive and have the entire hd set to C: ).... set your hardrive up into 2 halfs.. say make the first partition for WINDOWS and programs.... 3/4 the hardrive space.. .and the rest for BACKUP/Downloads ... this way reinstalling windows is a breeze.... with a relatively decent machine nowadays... you can do a format of the C: and fresh install of everything within 4 hours... I usually do this ONCE a month... but it's been well over 2 months now.. i'm just waiting for the new 4.6 omegas to be released

I've seen this before
There is some sort of odd rare problem were UT2004 has corrupted it self it’s not driver specific but I think it is driver triggered. By "driver triggered" that I mean the corruption starts when you switch drivers. I experienced that problem with the UT2004 beta demo. I haven’t seen it since the retail version.
The only solution I’d found with the one time I experienced it.
Do a complete uninstall of UT2004, then manually delete the c:\UT2004 directory then for good measure delete all files in your C:\windows\prefetch\ reboot and then reinstall the game.
See it that works for you…
