You say you have changed the psu, but what is the psu you are using now (brand and model)? And how many amps are on the 12V rail/combined 12V rail?
Despite that the HD2900GT is a cut down version (lesser shaders and got 256bit memory instead of 512bit memory, 256MB version only) of the HD2900PRO or XT, it still is a power hungery gfx-card, especially for the graphical power it can deliver. The newer HD3850 is a much power efficient gfx card with better performance, and nothing has been cut (all TMUs, pipelines, shaders are intact). For the gfx-card HD 2900GT alone you may need 135 watts at full load in 3d games.
It is recommended to use a power supply with at least 28-30 amps on the 12V rail (combined 12V rail) in your case. And that's not simply adding the amps of the 2 rails together; you have to check the max output of the 12V rail of the psu instead.
Update:
I see you are using the LOGISYS Computer PS575XBK ATX12V 575W PCI-E Power Supply which have on paper 25 amps on the 12V which is, I am afraid, the LOGISYS psu is not efficient for the HD 2900GT videocard. A budget psu as the mentioned one can never deliver the power that's printed on the lable of the power supply unit. If I were you I would immediately return the power supply if you can and buy a more decent psu which doesn't have to be expensive. A more decent psu costs around the 50 US dollars. You can get e.g. the OCZ StealthXstream 500W for around 60 bucks, 39.99 USD after rebate. The OCZ StealthXstream is a decent value psu which can power up your system with the HD2900GT which can deliver the power on the lable; despite the ripples of this unit is higher than the higher end power supplies the ripples are still within atx specification; it is a very nice power supply for this price, especially this OCZ unit includes active pfc. But ofcourse there are much better psu's out there but also much more expensive, e.g. the Corsair VX450 which is a Seasonic Oem with high-end components like Japanese capacitors etc..
I would get the Corsair VX450, since it's a really good power supply for the money (after rebate 59.99 USD), but if you can get the rebate for the OCZ StealthXStream 500W, then the OCZ unit is not a bad choice;
Here the link for the Corsair VX450 (free shipping buy.com): Corsair VX 450w Power Supply - CMPSU-450VX - Buy.com
Good luck!

Reason: adding the link for the OCZ StealthXstream 500W which I forgot.

I'm betting that the PSU is giving up the ghost. It's getting some age on it, and according to the specs it only had 25A on the 12v to start with.
As a PSU starts gettiing warmer, it's power output starts dropping, and then add to that when you start a game, the card asks even more from the PSU, and when it output drops far enough...crash.
$25.00 for a "575 Watt" PSU isn't going to get you much.
Do yourself, and your system, a favor and put in a Corsair VX450 or VX550.
Don't bet the life of your $180.00 Vid card on another $25.00 PSU, it just isn't worth it.
